Our online referral form is a simple way for whānau and services to connect with Ngāti Rangi support and services, ask for support, request connection, or highlight needs for an individual or whānau who may benefit from services, programmes, or tautoko provided by our organisation.
Whether you are referring yourself, a whānau member, or someone you are working alongside, this form helps us understand what is happening, identify areas where support may be helpful, respond in a timely, appropriate and safe way and match you with the support and kaimahi you need.

Through Ngati Rangi, whānau can access a range of kaupapa and services, including:
- Health services supporting overall hauora
- Whānau Ora services, whole-of-whānau support and planning
- Tamariki Ora, support for pēpi, tamariki and their caregivers
- Drug and alcohol support, confidential, non-judgemental support
- Hapū māmā, care and support during pregnancy and beyond
- Tāne Ora, wellbeing support for tāne
- Housing support, help navigating housing pathways
- Financial education, building confidence with pūtea and planning
- Kaumatua support and navigation
- A range of other programmes, intitiatives and services

How the referral process works
- Complete the online referral form and provide as much information as you are comfortable sharing
- Once submitted, the referral is sent directly to the right team
- Our kaimahi will then be in touch to follow up and kōrero next steps.
The information shared allows us to work together to create a clear picture of need, while respecting the mana, privacy, and voice of the person or whānau being referred.
